Prerequisites
It is required adequate knowledge of human anatomy, physiology and pathology, as well as pharmacology, laboratory medicine and diagnostic imaging. In particular, the student must be aware of the physiology of organs and systems (cardiovascular, gastrointestinal, nervous, respiratory, endocrine/exocrine glands, urogenital, hematopoietic), the pathological bases of diseases (inflammation, healing mechanisms, carcinogenesis) and the main organ diseases (e.g., heart failure, COPD, anemia, colitis and gastritis, endocrine gland dysfunction, autoimmune diseases). Finally, the student must be able to take a complete medical history, including physiological, pathological and pharmacological history and to collect a careful physical examination.
Learning outcomes
The aim of the Internal Medicine course is to teach the diagnostic and therapeutic approach to the internal medicine patient, with particular reference to the aspects of differential diagnosis and their complexity.
The student will be able to achieve this goal by using all the knowledge acquired so far.
Course contents
For each individual clinical situation involving internal medicine, students will be provided with the tools necessary to raise a diagnostic suspicion on the basis of anamnestic data and physical examination collected at the patient's bedside, and to utilize and interpret the most common biohumoral and instrumental investigations useful to confirm such suspicion. The student will be provided with the tools necessary to develop a patient tailored therapeutic plan, both pharmacological and surgical. All diseases of internal medicine interest are included in the program of the course which therefore should not be considered limited to the topics covered in the lectures.
Teaching methods
Student-teacher lessons with discussion of clinical cases. Main topics covered: gastroenterological diseases in the internal medicine, anemias and their etiology, chronic inflammatory intestinal diseases, diarrheal syndromes and constipation, malabsorption and malnutrition, abdominal pain and gastrointestinal bleeding, weight loss. The course will also be integrated with the discussion of clinical cases in the internal medicine.
Reccomended or required readings
Harrison: Principi di Medicina Interna. Eds Mc-Graw Hill Companies.
Rugarli Claudio: Medicina Interna Sistematica. VII Edition. Eds Edra Masson.
Assessment methods
Oral exam with open questions in order to assess the skills of diagnostic and therapeutic approach to the internal medicine patient, with particular reference to the aspects of differential diagnosis. The exam questions will not concern individual diseases, dealt with in previous courses and exams, and will not be limited to just the topics dealt with in the student-teacher lessons. Examples of exam questions: approach to the patient with anemia, approach to the patient with dyspnea, approach to the patient with shock, approach to the patient with diarrhea, approach to the patient in a coma.
The final grade of Clinica Medica 2 will derive from the weighted average of the grades obtained in the following modules: Internal Medicine A (1 CFU), Territorial Medicine (2 CFU), Geriatrics (2 CFU) belonging to the IC of Clinica Medica 1 of the V course year, and Internal Medicine B (6 CFU) and Emergency Medicine (2 CFU) of the sixth year of the course. The exams for the individual modules can be taken in separate sessions. There are no deadlines for individual exams passed.
