WRITTEN TEST
Prerequisites
Knowledge of the functional anatomy of the skull and of the physiopathology of the stomatognathic apparatus. Have attended the lessons of Gnathology with a basic knowledge of the fundamental elements of occlusodonzia and of the static and dynamic of the stomatognathic apparatus
Learning outcomes
The aim of the Prosthesis module is to acquire the theoretical and practical knowledge underlying the diagnosis, the measures aimed at the prevention or treatment of the pathologies of the individual teeth, of the supporting tissues and, more generally, of the structures of the stomatognathic apparatus with the objective to preserve or rehabilitate its functions in different age groups. Knowledge of the physiology of mastication, phonation and occlusion in relation to and relationship with prosthetic rehabilitative principles, especially with total removable items. Acquire adequate knowledge on the causes and consequences of partial and total edentism.
Knowledge of the fundamental bases for a correct clinical reasoning in relation to the patient's needs, desires and motivation; learn a correct approach to planning of total removable prosthetic treatment (first visit-diagnosis-prognosis-treatment plan).
Course contents
Terminology, classification and general information on removable prosthesis: total removable prosthesis, partial denture or removable partial denture with hook retention, removable partial denture with retention on precision attachments or p. joint
Causes of tooth loss and its local, regional and systemic consequences
Classification of edentulisms (Kennedy Classification and Applegate modifications).
TOTAL REMOVABLE PROSTHESIS
Prosthetic patient management: First visit, the medical record, medical and dental history, outline of the oral ecosystem
evaluation of temporomandibular function in the prosthetic patient, clinical and instrumental objective examination (radiographs, photostatus and diagnostic models, etc), clinical diagnosis and treatment plan, patient information, follow up.
Retention and stability in total prosthesis
Pre-prosthetic surgical preparations of the oral cavity
• Bony sharpness and sharp ridges
• Undercuts
• Excessively acute and painful miloioidea crest
• Excessive mucoperiosteal mobility of the alveolar ridge
• Excess of fibromucosa on the maxillary tuberosity
• Removal of the labial frenulum with too low attachment
• Lingual frenulectomy
Execution of the first upper and lower impression and analysis of the models
Marking of the edges on the upper and lower imprints and subsequent construction of the individual tray
Individual impression tray borders
Final upper and lower impression
Intermascular reports recording:
• Construction of resin bases with wax valleys
• Test of the wax top of the superior using the Fox plane
• Facial arch and articulation assembly of the upper model
• Choice of upper anterior teeth (shape, size and color)
• Assembly of the upper teeth (with back wax rim)
• Test of the lower wax vallum
• Assembly of the lower teeth (with back wax rim)
• Determination of the vertical dimension and of the intermaxillary ratios
• Phonetic tests
Assembly of models in the articulator
Replacing the back wax valleys with the missing elements
Clinical examination of the occlusion in the studio with the teeth mounted in wax and possible corrections
Delivery of the prosthesis to the patient
Design of a total prosthesis according to the Pound method
• Functional aesthetic criteria
• Phonetic tests
• Pound rule for the determination of the verticent
• Criteria for the choice of dental elements through the Pound rule
Design and construction of a total prosthesis according to the Gerber technique
Elements of design according to the techniques of Preti-Palla, Vergnano-Schreinemakers, R. Slavicek.
Differences and integrations between the various techniques
The biomechanics of the total prosthesis and the influence on osteomucous remodeling
Teaching methods
The teaching of this module consists of lectures of the teacher with the help of videopresentations with details and diagrams on the blackboard. Students are also suggested to discuss topics independently and then present and discuss together.
Reccomended or required readings
Lecture notes
Assessment methods
Learning is verified during the course with direct questions to the students during the lessons. At the end of the course the final assessment takes place through a written exam with open questions. The vote is expressed in thirtieths.
